Occupational therapy is a health profession that focuses on promoting health and wellbeing, by enabling people to participate in the activities of everyday life. Occupational therapists work with people to identify barriers to participation and strategies to overcome these barriers so people can regain their ability to participate in the everyday activities they want or need to do.
They do this by assessing a person's functional skills and capacity (such as their movement/strength, sensation, thinking skills, vision and perception) and comparing these to the demands of the task/activity itself and the environment in which they complete it.
Once this has been established, an Occupational Therapist can help a person regain or continue participation in their daily activities by looking at ways to make the task less demanding, assisting people to learn new skills or regain lost skills to improve performance of the activity or modifying the environment to accommodate changed personal functional capacity.
Driving is a complex everyday activity that many people value highly. It requires the ability to monitor and adapt rapidly to a wide range of different situations and environmental factors.
A person’s ability to participate in driving can be influenced by:
- accident, injury or illness
- congenital disabilities
- mental health disorders
- substance use
- factors associated with ageing, such as deteriorating eyesight
- lack of information about other transport options
When one of the above situations is present, decisions regarding a person's ongoing ability to drive safely are based on:
The OT driver assessment includes assessment of a driver’s sensory, physical and thinking skills required for driving to determine what symptoms may impact on safe driving. This is done initially through an off road assessment to screen for changes or difficulties in these areas followed by an on road driving assessment to observe how these changes actually present during performance of the driving task.
If difficulties and safety concerns are present, the occupational therapist will then use the information gathered to identify ways in which to assist the driver to continue or return to driving after a change in their previous function, if possible.
As part of the driver assessment, the occupational therapist will often make recommendations about driver rehabilitation, vehicle modifications and/or license conditions..
A comprehensive assessment of your medical and driving history, physical strength, movement, sensation, vision, and thinking skills that may impact driving followed by an on-road assessment to determine the impact of your medical condition or injury on your ability to drive safely.
The pre-drive/off-road assessment will take approximately 90 minutes. The therapist will take a full client history and discuss their need for driving, and they will assess the client’s sensory, physical and thinking skills for driving.
The on-road assessment is a practical assessment of a client’s driving skills, conducted in a dual control vehicle with the OT and a driving instructor in the car. The on-road assessment generally takes 50-60 minutes with feedback provided midway and at the end.
On-road assessments can vary in structure depending on the person's driving goals (unrestricted vs. local area only) and whether new vehicle modifications are being trialled (generally shorter in a quiet area to try different controls).
Recommendations regarding driver rehabilitation, vehicle modifications and/or license conditions are identified and implemented as required. A report is completed summarising the outcomes of the driver assessment and recommendations which are then communicated with the referring medical practitioners and Department of Transport as needed.
When vehicle modifications are required the on-road driving assessment outcome is 'not safe to drive' as alternative controls are needed to drive safely.
The process is then as follows:
Development and implementation of a driver rehabilitation program to help regain or strengthen skills impacted by a person's medical condition or injury. This may include rehabilitation of cognitive skills such as reaction time, observation of the environment or vehicle control. It may also include learning and practice in using different vehicle controls such as hand controlled accelerator and brake.
Assessment of a person's barriers and difficulties when undertaking driving lessons and development of a learning pathway with specific strategies to assist with the learn to drive process.

Recommendations vary depending on the goals, driving performance and safety of the person as observed during the on-road assessment. These can include:
The OT can only make recommendations based on the observations during the off-road and on-road driving assessments and these recommendations are then conveyed to Transport Tasmania and the client's GP.
The ultimate decision regarding licencing decisions sits with the client's GP and Transport Tasmania Licencing Department.

The hourly rate for an OT driver assessment is based on the NDIS price guide for private OT consultation which is currently $193.99/hr.
This includes both direct and indirect time such as organisation of mutually agreeable times for the on-road driving assessment between the client, OT and driving instructor and documentation/report writing and correspondence with relevant departments and health professionals.
If travel to your home is required for completion of the assessment, this may cost extra but all effort will be made to see several clients in the same area on the same day to keep these costs to a minimum.
A set cost subsidised fee is available for clients paying privately to help keep the service affordable for those without funding support. Please contact me for further information.
Some Private Health Funds include OT assessment as an extra in their policies. If your particular fund and level of cover includes this you can claim a rebate as determined by your particular Health Fund. Please check the details of your individual provider and cover for specific rebate amounts if this is applicable to you.
If your goal is to return to unrestricted driving, the assessment will generally be completed in and from the office in South Launceston as a set route on road assessment is required and it is expected that drivers can drive competently in any area. If you are absolutely unable to attend the office in Launceston for this assessment please contact me to discuss alternative options.
If your goal is to retain a local or restricted area licence only then the off road assessment can be completed in your home and the on road assessment will be completed in your local area.
